Ideally, you want the coupler sitting flat in direct contact with the top of the trailer tongue. If the gap between the bottom inside of the coupler and the trailer tongue is pretty minimal, say 1/16 of an inch or less, having it the way it is would most likely not cause a problem, but I would recommend you go ahead and fix it.

There will be no safety issues and you can absolutely back up with the Lock N Roll articulating coupler, no matter which one you use. Backing straight up won't be problematic and it will even be a bit easier to maneuver when turning due to the articulation. I can absolutely explain how the etrailer Trailer Coupler Lock for your flat lip coupler works, and it is extremely simple.
What happens is you will slide the lock over your flat lip coupler, and push the pin up - it won't take any strength at all, you can do it with a finger. If you install an aftermarket flat-bed that is 6-7 inches higher than the stock bed, you would need to create a recessed pocket for your gooseneck hitch to be used which many of the aftermarket companies account for. To connect the electric lockout on the Dexter Trailer Coupler, # 099-175-20, to your truck you will need to make sure the center pole of the 7-Way connector on your vehicle is wired to the reverse light circuit.

What you are describing is called a wishbone harness. This is different than a 5-Way flat connector like # A40W5B that you have referenced. A wishbone harness will have 5 wires, 2 of them going to the running light pin which is right next to the ground pin. This is so you do not have to splice in a jumper wire to go from one tail light to the other for the running light function.

What is going to be best for disc brakes when they are 40 feet away and the actuator is mounted at the front of the trailer is larger brake lines.
